Bellarmine Athletics is focused on creating for its student-athletes' opportunities that will become an integral part of the education and development of the "whole student".   Bellarmine Athletics also strives for championships, and our staff is dedicated to helping the Knights remain healthy and endeavoring toward their goals.

To help all of us reach our goals, an essential tool for the Bellarmine Athletic Training staff is a complete medical history for each student-athlete. This provides critical information for us and our team physicians to better care for student-athletes. Bellarmine University and NCAA policies also require us to have a complete record of each athlete's insurance coverage and require that each athlete has health and accident insurance in force while competing for Bellarmine University.

Below you will find links to the policies and procedures that you as a student-athlete are responsible for knowing. You will be asked to sign as acknowledgment of receiving this information that is within these documents through our ARMS software. Please realize that these documents are legal and binding documents and should be reviewed and understood before signing. Contact Brad Bluestone, Assistant Athletic Director for Student-Athlete Wellness/Head Athletic Trainer, if you have any questions or concerns about these forms.
